How to get your medical records

Medical records hold information about you. They are also sometimes called health records.

You'll have separate records for any NHS service you go to including your GP surgery, hospital, dentist or opticians.

How to get your record depends on which record it is.

If you require copies of your GP medical record please contact our reception team and ask for an "Access to Medical Records" form. Once submitted it can take up to 28 days for your record to be ready to collect. If your request is complex it may take up to two months.

Under General Data Protection Regulations (GDPR) we will provide this information free of charge. We may charge a reasonable fee if your request is considered unfounded, excessive or if you request the same information more than one.
