If you wish to register with the practice you can print off and complete the registration form and new patient questionnaire below. Please take this to the practice along with photographic identification and proof of address.
We will need these forms fully completed and shown appropriate, in date identification before you can be registered permanently with the practice.
Each person registering is required to attend in person
If you are coming to stay in Scotland from overseas then other information will be needed. Please print off and complete the registration forms below and take this along to the surgery. The receptionist will inform you of what information we will need and will let you know if you are entitled to free access of NHS services.
